We will do our best to make every visit to the practice as pleasant as possible. You will normally receive a courtesy reminder the day before your visit to remind you of your appointment. On your first visit to the practice we will ask you to complete a new patient form and medical history declaration, it would be helpful if you could arrive 10 minutes early to allow time to complete this form. Please bring your latest medication list to your appointment every time.
The dentist will discuss any appropriate treatment options and costs with you. We will provide you with an estimate and a treatment plan before you leave. You can pass the treatment plan to your Health Insurer to assess the benefits they will pay back and how much your out-of- pocket costs will be.
